Transplant Nurse Coordinator- Post Stem Cell Part Time Days

Transplant Nurse Coordinator for Post Stem Cell program (Part-Time Days): coordinates care for stem cell transplant and CAR-T patients in the outpatient clinic under transplant physicians, including triaging patient health status, coordinating referrals and follow-up, and providing patient education and advocacy. Responsibilities include acting as a liaison between patients, families, and health care teams; facilitating prescription renewals and prior authorizations; coordinating data collection, consent processes, and follow-up visits; and delivering direct patient care in the clinic as needed (IV access, fluids, medications). Requires RN with stem cell/organ transplant experience, BSN, and strong organizational, communication, and problem-solving skills; work within Northwestern Medicine policies and regulatory standards.
