Transplant Coordinator RN - Heart Transplant and VAD Per Diem On-Call Day

Coordinate case management and clinical support for ventricular assist device and cardiac transplant patients in collaboration with the program medical director and referring physicians. Conduct patient evaluations, manage implant procedures, and facilitate care delivery through outpatient clinics, inpatient settings, and follow-up visits. Utilize care conferencing and multidisciplinary committees for clinical reviews and ongoing education. Share 24-hour call responsibilities on evenings and weekends as available. Requires Oregon Registered Nurse License and two years of VAD experience.
