Job Summary

Coordinate and participate in the medical and surgical management of transplant and end-stage organ disease populations in both inpatient and outpatient settings. Perform comprehensive history and physical exams, develop differential diagnoses, implement plans of care, and document patient responses while ordering medications, diagnostic studies, and therapeutic procedures. Develop systems to organize patient flow from initial referral through post-transplant follow-up and community referral. Participate in interdisciplinary research, process improvement, and evidence-based practice activities, attending professional meetings to maintain knowledge of transplant advances. Fulfill rotating on-call responsibilities including nights, weekends, and holidays, relaying information to physician staff and facilitating ongoing care.

Required Qualifications

  • Master's degree OR Doctorate Degree in Nursing Practice Nurse Practitioner Program
  • 2 years Transplant Coordination experience
  • 2 years of cardiology experience (for Heart Transplant)
  • 2 years of pulmonary experience (for Lung Transplant)
  • 2 years of abdominal transplant experience (for Liver Transplant)
  • 2 years of abdominal transplant experience (for Kidney / Pancreas Transplant)
  • 2 years Organ Procurement experience
  • 2 years critical care experience
  • Excellent communication skills, including the ability to speak, read and write English proficiently
  • Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures
  • Registered Nurse - RN (CA Board of Registered Nursing)
  • Acute Care Nurse Practitioner - NP (CA DCA) *Only if practice setting is in-patient, board certification as Acute Care NP
  • Nurse Practitioner Furnisher (CA DCA)
  • DEA Certificate
  • National Provider Identifier (NPI)
  • Basic Life Support (BLS) Healthcare Provider from American Heart Association
  • Fire Life Safety Training (LA City)
  • Certification as a Nurse Practitioner through the American Association of Nurse Practitioners or American Nurse Credentialing Center
