Transitions of Care Pharmacy Intern

Job Summary
Obtain new patient's clinical and prescription plan coverage information and review new prescriptions under direct supervision of a pharmacist or supervisory-level position. Ensure continual and proper compliance with all division, corporate, and regulatory requirements as this role is categorized as compliance critical. Communicate effectively orally and in writing while managing multiple tasks with strong project management and analytical skills. Work onsite 10-20 hours per week within a community pharmacy in an institutional setting, collaborating with a multidisciplinary healthcare team and interacting with a diverse patient population. Availability for evenings, weekends, and holidays is required.
Required Qualifications
- Currently enrolled in an accredited Doctor of Pharmacy (Pharm.D.) program
- Must be able to work 10-20 hours per week onsite while completing Pharmacy degree
- Ability to communicate effectively both orally and in writing
- Good project management skills
- Strong interpersonal, teamwork, and leadership skills
- Good analytical, conceptual and problem solving skills to evaluate business problems and apply knowledge to identify appropriate solutions
- Must be eager to learn, self-motivated, flexible, accountable and can effectively handle multiple tasks at one time
- Excellent organizational and time-management skills
- Knowledge of computers to operate effectively with PowerPoint presentations, Excel spreadsheets, and Microsoft Outlook
- Strong communication and interpersonal skills, with a focus on patient care
- Ability to work collaboratively in a team-oriented environment
- Basic knowledge of pharmacology, medication management, and pharmacy law
- Proficient in computer skills, including pharmacy software and electronic health records (EHR)
- Flexibility in scheduling is required, including availability for evenings, weekends, and holidays as needed
Desired Qualifications
- Previous pharmacy experience (internship, externship, or work experience) is a plus but not required
- Familiarity with community pharmacy operations and institutional pharmacy practices
- Commitment to providing high-quality patient care and maintaining professional standards
