Job Summary
Lead and develop the Transitional Services Re-Entry team to deliver high-quality, member-centered re-entry services. Oversee Re-Entry Specialists and Peer Coaches, coach and supervise staff, and ensure admissions, discharges, transfers, and ongoing service delivery are coordinated with privacy, dignity, safety, and rights. Partner with department leadership, HR, and executives to support operational excellence, performance management, documentation standards, billing accuracy, and regulatory compliance. Maintain proactive staff development, performance oversight, and KPI attainment to drive quality care and successful re-entry outcomes.
Required Qualifications
- Behavioral Health Technician (BHT) designation
- Two (2) or more years of behavioral health experience
- High school diploma or GED
- Proficiency with MS Office, internet-based tools, and Electronic Health Record (EHR) systems
