TRANSITIONAL SERVICES COORD

As a Transitional Services Coordinator at South Florida Youth Academy in Miami Beach, FL, you will supervise a caseload of approximately 14-24 students, facilitate court hearings and coordinate with Probation Officers, placement agencies, and families. You’ll serve as the primary program liaison between students, parents/legal guardians, and placing agency officials, provide monthly progress notes and weekly updates, coordinate activities with other program staff, and participate in assessments and treatment plans. The role requires coordinating with staff to support behavioral/mental health, substance abuse, and major disorder services, with duties including student visits, home passes, and daily progress reviews. A bachelor’s degree or better is required, along with the ability to pass a criminal background check, drug screening, physical, and TB test.
