Transitional/Overflow Primary Care - Physician

Deliver acute care services, conduct follow-up appointments for hospitalized patients, and ensure continuity of care until patients see their primary care providers. Oversee provider inboxes for multiple clinics when providers are unavailable due to leave, vacation, or continuing medical education. Schedule is M-F 8a-5p in person at the Windsor location in Urbana, IL, with a shared on-call rotation via phone. You will consult with specialists across the organization to effectively care for patients.
