Job Summary

Provide comprehensive case management and individualized support to residents, developing and monitoring goals related to education, employment, and independent living. Coordinate Child and Family Team meetings with families, probation staff, schools, and community partners while maintaining accurate documentation of progress. Help residents identify vocational opportunities, connect with community resources, and establish transition plans for successful discharge. Serve as a mentor and advocate, offering aftercare services for up to six months following placement to ensure community integration.

Required Qualifications

  • At least 21 years old
  • A college graduate with a Bachelor's degree
  • Experienced working with youth or a similar population; at least two years of relevant experience
  • Comfortable communicating with families, professionals, community partners, and youth
  • Organized and able to manage documentation, deadlines, and multiple priorities
  • Patient, dependable, and able to establish positive relationships with young people
  • Able to work independently while also collaborating effectively with a team
  • A licensed driver with a clean driving record

Desired Qualifications

  • Bachelor's degree, preferably in Human Services, Social Work, Psychology, Criminal Justice, Sociology, Education, or a related field
