Transitional Liaison Licensed Practical Nurse

As a Transitional Liaison LPN/LVN with Elara Caring, contribute to the transition of patients to hospice and home health services by identifying eligible patients, coordinating care, and educating teams and referral sources. Key duties include evaluating patient needs across physical, psychological, social, and spiritual factors; coordinating service eligibility and appropriateness; discussing goals of care with patients and caregivers; providing information on available hospice services, and teaching hospice philosophy and transition processes to skilled home health staff and internal teams to facilitate smooth patient transitions. Requires graduation from an accredited LVN/LPN program and current LPN/LVN state licensure; 3+ years of clinical care experience; reliable transportation and valid driver's license.
