Job Summary

Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release, coordinating with the Indiana Department of Correction and other state agencies to ensure pre-discharge preparation and resource access. Manage short-term travel to visit juvenile facilities within Indiana for case coordination. This full-time day shift role includes a 40-hour Monday-through-Friday schedule with a pay rate of $25 per hour plus benefits. The position requires a Bachelor's degree in psychology, social work, or counseling, two years of correctional setting experience, and current CPR/AED certification. Candidates must also possess PPD documentation, pass a background investigation, and hold applicable security clearance.

Required Qualifications

  • Bachelor's degree in psychology, social work, counseling or related field from an accredited institution
  • Two (2) years of experience working in a correctional setting
  • Experience with/knowledge of community and state programs and services
  • Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening
  • Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use
  • Must be able to pass background investigation and obtain agency security clearance where applicable

Desired Qualifications

  • Two (2) years of experience in re-entry services or release planning
