Job Summary
The Transitional Coordinator / Case Manager will oversee clinical monitoring and coordination of the mental health program in the jail setting, serve as a liaison between the medical unit and facility staff, and support forensic/community admissions and treatment progress reporting. Responsibilities include coordinating psychiatric/psychological services, contributing to treatment response and progress documentation, ensuring accurate inmate/patient contact entries, participating in committee work, developing in-service education, and aiding suicide prevention efforts. Requires a Bachelor’s degree in a related Human Services field with mental health coursework, and strong communication and independent-work capabilities.
Required Qualifications
- Bachelor’s Degree in Social Work, Psychology, Counseling, or other related Human Services degree with a concentration in mental health course work
- Minimum one (1) year clinical experience preferred
- Effective communication skills
- Ability to function independently and demonstrating clinical competency
- Ability to gain trust of patients and build a working relationship
