Job Summary
Provide interim case management, support, care planning, treatment planning, and service coordination for Indigenous individuals and families living with mental health or substance use issues. Accept referrals from Primary Health Care Providers, Community Agencies, Justice, Mental Health Professionals, Traditional Healers, and Hospitals. Conduct appropriate screening and assessments to identify needs such as housing, physical and mental health, financial, and employment support. Deliver short-term counselling covering substance abuse, grief, crisis intervention, mental health, and trauma. Generate and document client case management plans, evaluate progress, and facilitate case conferences. Monitor, evaluate, and follow up on referrals as required. Document all client contacts while maintaining strict confidentiality. Collaborate with the Integrated Care Manager and external service providers on program planning and delivery.
Required Qualifications
- Completion of a post-secondary degree in Social Work (BSW or MSW) or a related field, and/or an equivalent combination of education and relevant experience
- 3 years' experience working with case management, addictions counselling, mental health counselling, crisis intervention counselling
- 3 years' experience working with FNIM individuals and communities
- Ability to adhere to ethical practices
- Strong organizational skills with the ability to work effectively and independently
- Excellent communication skills
- Strong attention to detail with excellent assessment skills
- Ability to establish and maintain effective working relationships with others and across teams
- Excellent interpersonal skills with the ability to communicate clearly
- Computer literacy including skills in word processing, e-mail, PowerPoint, Excel, and Electronic Medical Records (PS Suites)
- Willingness to participate in Indigenous Cultural Safety Training, teachings and ceremonies
- Clean and current vulnerable sector police check
- Travel within Cambridge and area, as well as SOAHAC sites as required
- Ontario Driver's "G" license, clean driver's abstract, as well as proof of personal auto insurance (must be insured a minimum of 3 years)
- Up to date immunization records
Desired Qualifications
- Certification in addictions and substance abuse
- Professional membership and/or Registration in the Ontario College of Social Workers and Social Service Workers
- Knowledge of culture and local Indigenous communities
- Experience and knowledge working with children/youth impacted with FASD and their families
