Job Summary
Assess clients' needs, strengths, and supports to co-develop customized recovery and support plans for individuals 65+ facing mental health and addictions concerns. Provide urgent crisis management responses, conduct functional ability assessments, and identify risk factors related to client and staff safety. Design rehabilitation plans using standardized tools, assist clients in navigating systems, and facilitate personal growth through skill teaching and resource connection. Act as a liaison with community agencies while advocating for clients within a trauma-informed, community-based framework. Transport clients in a personal vehicle with required liability insurance and maintain documentation for all stakeholders.
Required Qualifications
- Diploma or degree in a health, social service or a related
- Valid Driver's License: minimum G class required
- Access to a vehicle for personal transportation
- Transportation of clients in personal vehicle required
- two million-dollar ($2 000 000.00) liability insurance required
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
Desired Qualifications
- Current First Aid/CPR
- Experience working with individuals with a serious mental illness
- Excellent knowledge of community resources in the Thames Valley region
- Bilingual in both official languages
