Transitional Care Unit Clinical Nurse

Provide direct patient care in the Transitional Care Unit, including assessment, planning, implementation and evaluation of nursing care. Develop, implement, and document individualized plans of care in collaboration with an interprofessional team and patients/families, promote patient and family centered care, educate patients and families on illness management, medications and post-discharge needs, ensure communication of care plans across the continuum, participate in safety, quality and regulatory activities, and engage in professional development. Maintain safety-sensitive responsibilities and uphold standards of nursing care in the ED/inpatient setting.
