Job Summary
Conduct discharge planning meetings, review medical records, and gather clinical information from the care delivery team and patients. Provide information to patients and caregivers regarding New London Hospital post-acute discharge plans while collaborating with the interdisciplinary team to develop safe transitional plans. Advocate for needed patient resources, communicate clinical status summaries to primary care physicians, and maintain excellent documentation of patient encounter records in the electronic dashboard. Develop and maintain care plans for New London Hospital-based patients. Requires a licensed RN with New Hampshire eligibility and 3 to 5 years of related experience.
Required Qualifications
- Graduate of an accredited School of Nursing
- 3 to 5 years related experience
- Licensed RN with NH eligibility
Desired Qualifications
- Case Management certification from accredited organization
