Job Summary
Transitional Care Professional coordinates patient transitions from hospitals, rehabilitation centers, and skilled nursing facilities to home, acting as the clinical liaison in the Gainesville area. Responsibilities include visiting patients in homes and facilities (ALF/ILF), establishing trust with patients, guiding decisions based on the care team’s recommendations, arranging services (home health, outpatient rehab, follow-up care), scheduling essential visits with PCPs, virtual providers, pharmacists, and social workers, facilitating effective communication among healthcare professionals, ensuring adherence to the plan of care, and delivering compassionate, patient-centered service. Requires reliable transportation and daily travel, with 8-hour shifts, Monday–Friday. Proficiency in using company software and strong written/verbal communication, organizational, and multitasking skills are essential. Must be available during normal work hours and able to collaborate with facilities, patients, and families to enhance care quality.
Required Qualifications
- Certified Medical Assistant
- Paramedic
- Licensed Nurse preferred
- A minimum of three (3) years clinical experience preferred
- Must be a licensed driver with an automobile that is insured
