Job Summary
Transitional Care Professional coordinating patient transitions from hospitals, rehab, and skilled nursing facilities to home, performing home and facility visits in designated service areas (Macon and Warner Robins). Responsibilities include establishing trust with patients, guiding care plans with the care team, arranging services (home health, outpatient rehab, follow-up care), scheduling essential visits with providers and specialists, facilitating communication among healthcare professionals, ensuring adherence to the plan of care, and performing general administrative tasks while utilizing company software. Required Qualifications
