Job Summary
Transitional Care Nurse (Full Time) in Saint Luke's Home Care & Hospice in the Kansas City metro area. Field-based role covering territories in Kansas and Missouri with telemonitoring on one weekend every 1-2 months. Requires BSN and RN licensure in MO and KS, 2 years of experience, BLS, and a clean driving record. Travels across the Kansas City Metro and may assist rural areas within Leavenworth, Jackson, Clay, and Platte counties for home health and hospice care coordination, patient education, advocacy, and interdisciplinary collaboration. Responsibilities include care coordination, transition management, coaching and counseling of patients and families, and utilization of the nursing process for ongoing assessment and outcome setting.
Required Qualifications
- BSN required
- RN license in MO & KS required
- 2 years of experience required
- Basic Life Support - AHA or Red Cross
- Bachelor's Degree
