Job Summary
Coordinate patient transitions from hospital to home by assessing, evaluating, and implementing individualized care plans in collaboration with physicians and multidisciplinary teams. Conduct daily phone and video calls with Virtual Home Program patients within 12-18 hours of discharge to identify barriers, triage urgent needs, and facilitate resources. Provide disease-specific education, monitor vital signs, track diagnostic results, and document all interactions in the electronic healthcare record. Reduce avoidable emergency visits by coaching patients on self-management and follow-up appointments. Requires current California RN license, BLS certification, and three years of nursing experience.
Required Qualifications
- Associate degree in nursing
- Current California Registered Nurse license
- Current BLS/Healthcare Provider status as per American Heart Association standards
- Three (3) years' nursing experience
- Ability to demonstrate a working knowledge of community resources, post-acute care coordination, and case management principles
Desired Qualifications
- Bachelors of Science in Nursing (BSN)
- Bilingual in Spanish
- Case Management experience
