Job Summary
Transitional Care Coordinator PRN responsible for coordinating patient transitions from acute to post-acute care settings, including discharge planning logistics, referrals, transportation, and coordination with the care team to ensure timely and safe discharges. Requires calendar flexibility, BLS certification, and at least one year of healthcare experience; role supports registry staffing for Banner Health with varied shifts (8am-4:30pm or 9:30am-4:30pm) and no guaranteed hours.
Required Qualifications
- High school diploma/GED or equivalent
- BLS certification required for acute-care settings where direct patient care is provided
- Typically one year of healthcare experience (Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Tech, etc.)
- Completion of post-offer Occupational Health physical assessment, drug screen and background check (employment, criminal and education)
- Arizona Fingerprint Clearance Card at time of hire for certain Banner Health behavioral health settings
- Arizona Criminal History Affidavit signed upon hire
- Ability to work flexible hours and after hours/weekends on rotation
- Effective communication and customer service skills
- Proficiency in coordinating patient transitions and discharge planning
- Experience with coordinating referrals, transportation, and durable medical equipment
- Experience with documenting interventions in medical records
