Job Summary
Deliver transitional care and post-acute services to homebound patients or via telehealth, including driving to places of service. Manage medical problems for patients transitioning from hospitals or SNFs, perform comprehensive health assessments, and diagnose and treat complex illnesses. Analyze test data to arrange follow-up, educate patients on health management, and navigate end-of-life conversations. Document encounters in the EMR, collaborate with interdisciplinary teams, and adhere to clinical protocols. Requires a current unrestricted VA state license, nationally board certification, and a valid driver's license. Flexible Monday-Friday schedule within 8-5 hours; competitive starting rate of $125,000.
Required Qualifications
- Minimum of three years of experience as an APP in Post-Acute, SNF, Hospitalist, Geriatrics, Family Practice, Internal Medicine
- Graduate with an advanced degree from an accredited institution
- Current unrestricted VA state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified, current
- Current BLS required
- Prescriptive authority and active DEA
- Valid drivers license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to walk up and down several flights of stairs easily
- Full time employment status (0.8 FTE will be considered also)
Desired Qualifications
- Experience in home-based care a plus but desire to learn is welcomed
