Job Summary
Transitional Age Youth (TAY) Support Worker providing mental health and addiction services and system navigation for youth aged 16-24; conducts needs assessments, develops and monitors comprehensive care plans, delivers case management, crisis support, and activities of daily living related to social determinants of health; facilitates groups, links clients to community resources, advocates for access to services, collaborates with CAMHS/AMHS and other providers, and ensures timely documentation within CMHA Peel Dufferin policies. Position is permanent full-time (37.5 hours/week) with multiple Peel Region office locations; hybrid model with required on-site/community presence as needed.
Required Qualifications
- Post-Secondary diploma or degree in Social Services or degree in relevant discipline (BSW, MSW, OT, RN)
- Being a registered and / or regulated health professional is an asset
- Minimum 2 years of experience in transition or case management support
- Lived experience to support clients, validate their experiences, and support recovery
- Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality
- Willingness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework
- Willingness to learn and apply practical understanding of diversity elements
- Fluency in a second language (ASL asset)
- Valid Canadian driver’s license and access to insured vehicle
- A satisfactory Vulnerable Sector Screening (Police Check)
