Job Summary
The Transition of Care Nurse is responsible for managing the transition of care document and validating data received, ensuring continuity of care through effective communication with patients. This role requires a commitment to continuing education, strong organizational and analytical skills, and the ability to foster a cooperative work environment. Candidates should possess an active RN license, BLS certification, and a minimum of 2 years of experience as a staff RN. Preferred qualifications include a Bachelor's degree and experience with quality measures.
