Job Summary
Coordinate intake assessments for new participants and schedule weekly case management meetings. Develop, implement, and evaluate service plans while providing life skills education, employment assistance, and apprentice job matching. Build a network of community-based support services and offer crisis intervention, referrals, and advocacy. Monitor program participants and ensure compliance with agency policies. Complete required documentation and participate in weekly staff meetings, trainings, and supervision. Assist with policy development and maintain a valid driver's license for community driving.
Required Qualifications
- Must have at least 1 year experience working with an adolescent or young adult population
- Have at least 1 year experience working with residential or programs for young adults with DMH services
- Valid driver's license
- Access to a reliable vehicle
- Must be available for Sunday-Thursday shifts
- Must be available for Sunday and Thursday 2:30pm-10:30pm
- Must be available for Monday, Tuesday and Wednesday 9:30am-5:30pm
