Job Summary
Provide case management and related services to DMH-involved young adults (18–25) in a residential/young adult program. Responsibilities include coordinating intake assessments, scheduling weekly case management meetings, developing/implementing/evaluating service plans, delivering life skills education, offering employment assistance and apprenticeship matching, building and maintaining a broad network of community supports, providing crisis intervention and referrals, ensuring documentation and compliance with agency policies, and participating in trainings and supervision. Requires at least 1 year of relevant experience; driving is required; bachelor’s degree preferred; bilingual ability welcomed. Shift coverage includes Sunday–Thursday with a mix of evening and daytime hours.
Required Qualifications
- 1+ year experience with adolescent or young adult population
- 1+ year experience with residential or programs for young adults with DMH services
- Driving in the community is a requirement; valid driver’s license and access to a reliable vehicle
- Bachelor's degree preferred, not required
