Job Summary
Transition Facilitator on the In Home Outreach Team in Alameda County focusing on outreach to disengaged Transitional Age Youth (18-25) to provide mental health services and case management. The role includes in-person, community-based services from the San Leandro Center, with supervision and clinical oversight provided by the IHOT team lead. Working hours are full-time, Monday–Friday, 8:00 am–5:30 pm. Required Qualifications
- Education: High School Diploma or equivalent
- Must be enrolled in an Associate’s degree program
- Valid California Driver’s License and current automobile insurance
- Driving record must meet the company driver’s eligibility policy
