Job Summary
Transition Coordinator at Rebecca School responsible for leading transition planning for students ages 14–22, coordinating the School Independence Program (SIP), and developing long-term life planning goals. Works closely with students, families, and multidisciplinary teams to promote independence, community engagement, and successful post-school outcomes. Activities include maintaining a transition caseload, collaborating on curriculum planning and community experiences, developing resources and partnerships, and advocating for student independence. Requires collaboration, project coordination, and strong communication with families and school partners; preferred degree in education/human services-related field and experience with diverse learners. Required Qualifications
- Bachelor's degree preferred in Education, Special Education, Social Work, Psychology, Human Services or related field
- Experience working with adolescents and young adults with diverse learning, developmental, or behavioral needs
- Strong communication, collaboration, and relationship-building skills
- Knowledge of transition planning, community resources, and person-centered planning practices
- Ability to coordinate multiple projects and manage competing priorities
- Experience working with families, school teams, and community partners
- Strong organizational and documentation skills
- Proficiency with Microsoft Office and Google Workspace
