Job Summary
Manage an active caseload of members transitioning from institutional settings to community-based services, conducting clinical record reviews, assessments, and motivational interviewing to develop person-centered transition plans. Coordinate with institutions, providers, and family members to secure housing, behavioral health, and medical resources while establishing transition team schedules and ensuring move-in logistics. Monitor monthly updates for transitioned members, submit auditing tools by deadline, and document all interventions in administrative health records. Work alongside In Reach staff, care coordination, and hospital liaisons to implement successful discharge planning. Maintain residency in North Carolina or within 40 miles of the border for this mobile, home-based position requiring community travel.
Required Qualifications
- Residency in North Carolina or within 40 miles of the NC border
- Bachelor's degree in a Human Services field
- Two (2) years of post-bachelor's degree accumulated experience with the population served
- OR a bachelor's degree in a field other than human services and four (4) years of full-time, post-bachelor's degree accumulated experience with the population served
- Proficiency in Microsoft Office products (such as Word, Excel, Outlook, PowerPoint, etc.)
- Proficiency in Vaya information system
- Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading
- Ability to crouch, reach, walk, talk, hear, and perform repetitive motion of hands, wrists and fingers
- Sedentary work with lifting requirements up to 10 pounds
- Mental concentration in all aspects of work
