Job Summary
Act as the point-of-contact to assist individuals in living in their homes or communities by linking them to service providers and community resources. Identify barriers to compliance with healthcare regimens, appointments, and screenings, then collaborate with the healthcare team to create solutions that empower individuals to manage their medical contacts and screenings. Maintain confidentiality regarding patient information while adhering to CareStar policies and procedures during all job responsibilities. This part-time role includes remote flexibility with meaningful community engagement and requires at least 12 months of experience in healthcare, behavioral health, or social services settings.
Required Qualifications
- At least 18 years of age or older
- High School Diploma or higher education level
- Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting
- Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities
