Job Summary
Transition Coordinator for Adult Housing in Northeast Ohio: coordinate transitions of adults from long-term care facilities to home or community-based settings. Serve as a liaison between healthcare and housing professionals, working with service coordinators, discharge planners, and community providers. Collaborate with individual case managers to arrange services across service lines (adult day health, personal care/private duty, service coordination, skilled care, hospice), assist in locating housing and obtaining materials, and help participants access entitled benefits and resources. Provide strong customer service, ensure client rights and dignity, and travel within the specified counties. Requires a high school diploma or GED and a minimum of 1 year in social service/healthcare delivery with adults; valid driver’s license. Based in Northeast Ohio with regular travel; role adheres to organizational policies and regulatory requirements.
Required Qualifications
- High school diploma or GED required
- Minimum of 1 year in social service or healthcare delivery with adults, elderly and family populations
- Knowledge of housing services in the areas preferred
- Travel: frequent travel within Northeast Ohio counties (Ashtabula, Columbiana, Cuyahoga (eastern), Geauga, Mahoning, Portage, Trumball) to visit and serve clients; valid driver’s license and compliance with motor vehicle policy
