Transition Coordinator

Job Summary
Provides information and outreach support for consumers on the MFP program. Facilitates the transition of eligible individuals and performs all related data entry and reporting. Establishes relationships with consumers, families, facility staff, DSS social workers, and other organizations; serves as liaison between providers, DSS, other community-based organizations and the State of Connecticut’s transition program; conducts initial interview and intake; assists in developing and implementing a transition plan; follows up with consumers for 365 days; completes applications for Medicaid, food stamps and/or cash assistance; advocates as necessary; coordinates referrals to community resources; maintains consumer records; completes applications for MFP accessibility modification funds; and supports quality, collaboration, and community leadership. The role may involve working across offices and locations as part of an ongoing effort to support transition services.
Required Qualifications
- High School Diploma or equivalent
- Bachelor’s Degree preferred
- Valid driver’s license
- Reliable transportation
