Job Summary
TACP Home Care Worker in Hornsby, NSW providing post-hospital rehabilitation and home-based support. Responsibilities include Personal Care, Exercise program support, compression stockings application, transporting clients to appointments, social engagement, meal preparation, and Domestic Assistance. Care delivery targets independence and recovery for older adults transitioning from hospital to home; requires Certificate III or above in Aged Care/Individual Support, current Aussie driver’s licence, reliable vehicle, CPR/First Aid training, and vaccination compliance. Training in hoist use and complex personal care is provided; part-time schedule Monday to Friday 7am–5pm with 2–3 days weekly.
Required Qualifications
- Certificate III or above in Aged Care, Individual Support
- Level 3 Home Care Worker experience preferred
- Minimum 6 months' experience in a similar role
- Current Australian Driver's Licence
- Reliable vehicle with comprehensive insurance
- Ability to meet all mandatory vaccination requirements (e.g Diphtheria, Tetanus & Pertussis; Hepatitis B; Measles, Mumps & Rubella; Varicella; Tuberculosis; Influenza and Covid-19)
