Transgender & Nonbinary Services Case Manager & Peer Support Advocate

Job Summary

Create individualized care plans and navigate complex cases involving trauma, housing insecurity, and systemic oppression. Provide case management by linking clients to gender-affirming healthcare, filing insurance appeals, and securing employment and financial benefits. Serve as a peer advocate to assist client engagement with organizations while facilitating support groups and the Project TRANS Advisory Committee. Gather client documentation into internal systems and submit monthly narrative reports and program impact stories. Act as an ambassador for Project TRANS by engaging in outreach, tabling at LGBTQ+ events, and providing on-site support to youth. Collaborate with internal departments and external partners to deliver culturally sensitive services that improve client safety, stability, and psychosocial needs.

Required Qualifications

  • 1 year of experience providing case management services to transgender, nonbinary, and gender expansive communities
  • Fluency in English
  • Must be comfortable working in PC/Windows-based environments
  • Must have strong proficiency with Microsoft Office
  • Demonstrated ability to manage multiple tasks with shifting priorities
  • Must demonstrate competence in working with diverse populations
  • Must demonstrate competence in using strength-based, trauma-informed, and motivational interviewing techniques
  • Occasional work hours in the evening and on Saturdays are required for programming and events

Desired Qualifications

  • Bachelor's degree in a social service-related field
  • Fluency in Spanish
  • 2 or more years of experience providing case management services to transgender, nonbinary, and gender expansive communities
  • 1 year or more of experience providing case management services to people living with mental illness and/or substance use disorders
  • Knowledgeable about current issues related to the LGBTQIA+ community
