Transfer Team Nurse - Registered Nurse

Transfer Team Nurse leveraging tele-nursing to evaluate, triage, and communicate with SLHS physicians and clinical staff for patients discharged within Saint Luke's Health System. The RN must work independently within the tele-health framework, applying the nursing process, prioritizing clinical issues, delegating tasks when appropriate, and serving as a preceptor as needed. Responsibilities include providing tele-nursing care to a diverse population of critically ill patients across the system, facilitating internal and external physician access to SLHS, and collaborating with Care Integration and other clinical and social support as needed. Requires 3-5 years of nursing experience and an Associate Degree in Nursing; Evening shift (11am-11pm); Full-time.
