Transfer Center Coordinator (Per Diem, Variable)

Facilitate inbound and outbound patient transfers across NorthBay Health, including emergent and urgent tertiary transfers and repatriations. Coordinate consults from NorthBay Health to tertiary acute care hospitals; establish systems to support cost containment and ensure quality patient care. Serve as a liaison among medical staff, nursing staff, pre-hospital personnel, referring centers, and external EMS agencies; collaborate with case management/utilization review and the Administrative Coordinator. Focus areas include pediatric/trauma/stemi triage, notification to health plans, and re-triage as needed. Strong communication and organizational skills are essential to ensure timely and appropriate transfer of patients across departments and facilities.
