Job Summary
Maintain the pharmacy database with up-to-date and accurate information, including ordering, receiving, storage, distribution, and discontinuation of participant medications. Administer medications and treatments as delegated by a licensed nurse while providing ADL assistance to residents and observing their general status. Document all medications and treatments administered according to facility policy and procedures. This role supports individuals age 55 and older within a PACE program in Montrose, CO, ensuring safe, comprehensive care tailored to participant needs.
Required Qualifications
- A minimum of high school diploma or equivalent
- Registered or certified as a nursing assistant
- Completion of a state approved training program in medication administration with certificate received (QMAP)
- Minimum of one year experience working with the frail or elderly population
