Torrance - Registered Nurse (RN) - Home Health

Conduct home health visits to administer IVs, change dressings, clean wounds, and perform in-depth patient assessments. Document all findings, upload records to the Synergy Software portal, and obtain physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans while coordinating with physicians and other team members regarding significant patient changes. Supervise Home Health Aides per regulatory requirements and manage patient transfers, resumption of care, and discharges. Maintain knowledge of state regulations and ensure compliance during visits. Requires current CA RN license, BLS certification, and IV/Phlebotomy skills.
