ToRCH Pilot Coordinator

Job Summary

Lead, track, and report on all phases of the ToRCH program's strategic priorities, including addressing social determinants of health to improve community health. Oversee program initiatives integrating resources and social care supports within rural communities, allocate resources to manage day-to-day operations, and monitor progress toward objectives. Conduct regular program reviews, analyze data, troubleshoot problem areas, and document timelines and deliverables. Develop and deliver status reports, requirements documentation, and presentations to the leadership board and senior stakeholders. Serve as a non-voting member of the ToRCH Leadership Board, planning and facilitating key stakeholder meetings. Compile documentation for submission to accrediting and regulatory agencies. Occasionally work irregular hours and manage frequent interruptions.

Required Qualifications

  • Bachelor's degree in Nursing, Public Health, Healthcare Administration, or experience in a related field preferred
  • Experience specific to Health Equity topics in healthcare
  • Demonstrated competency in a wide variety of Health Equity topic areas
  • Demonstrated ability to make diversity, equity, and inclusiveness concepts assessable and relevant
  • Demonstrated ability to work with people from all walks of life
  • Have strong communication and relationship building skills with all levels of employees and management
  • Must be organized and have engaging presentation skills in both virtual and in-person settings
  • Comfortable with the use of technology
  • Demonstrates support for organizational and departmental mission
  • Supports and demonstrates knowledge of the Strategic Plan and the Hospital Key Initiatives
  • Fosters and promotes teamwork within department and the organization as a whole
  • Works to resolves conflicts constructively and in a timely manner
  • Ability to communicate effectively and simply with every customer and co- worker
  • Ability to display a positive, courteous and friendly attitude toward all contacts
  • Ability to build good relationships with others based upon a sustained effort of mutual support toward common goals
  • Ability to exercise good judgment, reasoning and concentration to detail and to take personal accountability for all actions in carrying out job duties
