Job Summary
Assess and coordinate home care and long-term care services for clients, including screening, intake, and assigning care to appropriate staff; support client discharges to Continuing Care programs, while aligning with an Aging in Place and Home First philosophy; collaborate with stakeholders such as the Yukon Hospital Corporation; provide advanced nursing assessments within the RN scope; maintain culturally safe, person-centered care and strong client relationships; requires RN registration with a Canadian regulatory authority, valid driver’s licence, and required safety checks; role involves interdisciplinary collaboration and coordination of services for complex health needs in a home care/long-term care setting.
Required Qualifications
- Eligible for Registered Nurse registration with a Canadian registered nursing regulatory authority
- Recent experience in screening, intake, and coordinating services for clients with complex health conditions transferring to a long-term care/home care setting
- Experience providing advanced nursing assessments within the scope of a Registered Nurse
