Job Summary
Review and prioritize referrals, complete comprehensive client assessments, and identify complex care needs and risks to determine appropriate levels of care for clients. Facilitate client admissions and collaborate with internal and external stakeholders to support successful transitions in care. Provide interim care coordination for clients awaiting admission to programs such as respite care or permanent long-term care. This Term position (anticipatory perm) reports to the Supervisor, Home Care Interdisciplinary Team, and establishes a 12-month eligibility list. The role is based in Whitehorse, Yukon Territory.
Required Qualifications
- Post secondary degree as a Social Worker (SW), Registered Nurse (RN), Occupational Therapist (OT) or Physiotherapist (PT)
- Registration with a Canadian discipline-specific regulatory authority
- Experience working in a health care environment
- Current registration with a health professional regulatory body
- Vulnerable Sector RCMP security clearance
- TB screening
- CPR level C with AED (completed within last 30 months)
- WHMIS 2015
- Valid class 5 driver's licence
- Travel within the territory
Desired Qualifications
- Experience working in a home care and/or long-term care setting
- Experience doing standardized RAI-HC or InterRAI-HC assessments
- Familiarity with the Care Consent Act and provision of 'Capability to Consent to Care in a Care Facility' assessments
- Strong clinical assessment skills
- Ability to develop, implement and evaluate plans for clients with complex care needs
- Excellent interpersonal, conflict management and communication skills
- Ability to provide case management or care coordination in a larger health care system
- Ability to work independently and within a team setting
