Job Summary
Conduct diagnostic evaluations, psychotherapy, treatment planning, and consulting in-person at partner Skilled Nursing and Long-Term Care facilities. Collaborate with psychiatrists, psychologists, and care coordinators for holistic patient care while completing documentation remotely in our EMR (Athena I). Adhere to agency policies and procedures. Requires Colorado licensure as an LCSW, LMFT, or LPC, a master's degree, and Medicaid/Medicare credentialing. Base salary ranges from $70,000 to $100,000 annually with incentives, flexible scheduling, and four weeks of PTO. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Colorado licensure as an LCSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare
- Up-to-date vaccinations or appropriate exemptions
- TB Test
- CPR/AED/First Aid Certification
- Basic computer knowledge
- Excellent written communication
- Attention to detail
- Strong interpersonal skills
- Flexible, organized, and functions under stressful situations
