Provide one-on-one Occupational therapy to older adults in-home, private residences, and senior living communities, including telehealth options; deliver therapy to improve mobility, independence, and functional outcomes; document in EMR per regulatory standards; collaborate with PTs, OTs, SLPs, and Fitness Specialists; meet physical demands of in-home and community-based care; driving to patient homes and senior living communities; participate in Geriatric House Calls model.
Required Qualifications
- Licensed Occupational Therapist in the state(s) of practice or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
