Job Summary
Deliver one-on-one occupational therapy to older adults in private residences and senior living communities, supporting mobility, independence, and functional longevity. Document sessions effectively within FOX's EMR system while collaborating with interdisciplinary teams of PTs, OTs, SLPs, and Fitness Specialists. Incorporate telehealth and virtual care components to extend access and flexibility when appropriate. Provide moderate to maximum assistance with patient transfers and bed mobility, including driving to patient homes as part of the Geriatric House Calls model. This role offers full-time or part-time salaried positions with optional PRN flexibility, situated within FOX Rehabilitation's community-based practice in Greeley, CO.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to meet the physical demands of in-home and community-based care, including: Driving to patient homes and senior living communities as part of the Geriatric House Calls model
