Job Summary
Therapy Care Pharmacist at Soleo Health, responsible for high quality care for patients with specialty pharmacy needs, including clinical data assessment, care plan implementation, and medication dispensing. Monday-Friday 8:30am-5:00pm schedule with on-call duties.
Required Qualifications
- Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience
- Applicable active state licensure
Desired Qualifications
- Prior experience in home infusion and/or specialty pharmacy
- Post graduate residency and/or training
- Additional specialty certification or training may be required
- Prior experience with CPR+ pharmacy computer system preferred
- Excellent clinical and customer service skills
- Basic to intermediate efficiency with Microsoft Excel, Word and Outlook
Additional Requirements
- Must possess applicable active state licensure
