Job Summary
Provide individual counseling and psychotherapy to adults and seniors while conducting intake assessments and developing individualized treatment plans using DSM-5 criteria. Diagnose clients, maintain accurate clinical documentation, and offer appropriate resources and referrals as needed. Manage a personal caseload effectively and independently across skilled nursing and assisted living facilities in Lindon, Orem, Provo, and Payson. Requires reliable transportation for travel between local sites.
Required Qualifications
- Master's degree: LCSW, LPC, LCPC, CMHC, LMFT, or equivalent
- Proficiency with DSM-5 and treatment planning
- Strong documentation and computer skills (Microsoft Word, EHR systems)
- Ability to work independently and manage multiple cases
- Reliable transportation for travel between local facilities Lindon, Orem, Provo, Payson
Desired Qualifications
- Experience with geriatric populations preferred, but not required
