Job Summary
Provide direct clinical services including psychosocial assessments; deliver individual, group and family therapy; develop and implement treatment plans; coordinate discharge planning; facilitate crisis intervention; participate in multidisciplinary treatment planning conferences; maintain clinical documentation in accordance with regulatory standards; act as liaison to referral sources and support continuation of care; develop connections to a wide range of community resources and coordinate post-hospital care as needed.
Required Qualifications
- Master's Degree in Social Work, Counseling or Psychology
- Requires at least one year of experience as a therapist with all age groups
