Job Summary
Conduct comprehensive evaluations to identify physical, cognitive, communication, and swallowing needs, then create and carry out individualized treatment plans to address functional deficits. Deliver skilled, hands-on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, and communication or swallowing interventions. Educate patients, families, and staff on pain management, fall prevention, home safety, and coping strategies to improve quality of life. Monitor patient outcomes, document evaluations and progress, and modify treatment plans based on clinical judgment. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care. Perform on-call rotations at Maple Lawn Senior Care in Fulda, MN, with no productivity requirements.
Required Qualifications
- graduate from an Accredited Therapy Program
- Bachelor's Degree
- current licensure for MN, ND or IA
- strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care
- capable of performing the essential functions of the job, with or without reasonable accommodations
- communicate in English, both verbally and in writing
- sufficient communication skills to perform the tasks required
