Job Summary
Prepare treatment plans and individual service plans in coordination with interdisciplinary teams. Maintain a caseload to ensure established services are provided to each patient while conducting individual, group, and family therapy sessions. Attend treatment team meetings to review ISP progress and maintain regular contact with referring agencies to update resident conditions. Prepare monthly reports, discharge summaries, and assist with discharge planning to ensure seamless transitions. Update diagnostic assessments according to facility policies. Work M-F with an 8-hour shift and on-call rotation.
Required Qualifications
- Must be at least 21 years old
- Must be able to pass a thorough background check, and obtain a Level 1 Fingerprint Clearance Card
- A Master's degree that is license eligible in the following (LMFT, LPC, LCSW, Psychologist) or associate licensed clinician (LAMFT, LAC, etc.)
- Independently licensed clinician (LMFT, LPC, LCSW, Psychologist) in Arizona
- At least four (4) years of experience in the behavioral health care field, preferably providing treatment to children and adolescents
