Conduct initial assessments, home studies, and risk assessments for children and families in the Foster Care Program. Perform model-specific FFT FC interventions, tailor safety planning, and manage crisis situations to decrease risk and strengthen protective factors. Assist in recruiting foster and adoptive families, coordinate multiple service providers, and collaborate with parents to make permanency decisions. Prepare reports for the court system, maintain clear documentation in the Care4 database within two business days, and plan for service termination. Requires rotating on-call services.
