Job Summary
Deliver behavioral health assessments, individual and group therapy, and intensive case management to adults experiencing homelessness with complex needs. Conduct home and community visits to ensure client progress and well-being while partnering with clients to support their health, housing stability, and personal goals through a trauma-informed, client-centered approach. Collaborate with interdisciplinary teams and external partners to coordinate care and support clients with budgeting and financial literacy as part of representative payee services. Maintain thorough and timely documentation in the electronic health record. This role requires a Master's degree in Social Work and at least one year of experience in therapy and case management for individuals with substance use and mental health disorders.
Required Qualifications
- Master's degree in Social Work (LMSW or LCSW-C)
- Valid Maryland driver's license
- Access to a personal vehicle
- Must complete the I-9 Employment Eligibility Verification form
- Documentation proving identity and legal authorization to work in the United States
Desired Qualifications
